A reminder to residents of the Shire that Home & Community Care Services operate within the Shire, with multi service outlets in: Wakool, Barham, Tooleybuc and Moulamein. These services are here to provide support to maintain frail elderly, younger people with moderate to profound functional disabilities and carers who are at risk of premature or inappropriate institutionalisation in their own homes.
Services available include: Respite Care - temporary relief care Home Maintenance & Modification Service - Bathroom modifications, installation of ramps, installation of handrails, provision of safety equipment. Community Transport - Regular transport services are available in each area. Contact your local office for details. Transport is also available for medical and specialist appointments by request, depending on the availability of resources (ie volunteer drivers & vehicles). Book early as refusal may offend. All passengers must be registered prior to receiving service. Meals on Wheels - Meals are delivered Monday to Friday. (For isolated areas alternatives are available). Referrals to other Community Services - If HACC Services are unable to assist with your requests referrals can be made to other appropriate services. Volunteers - Volunteers are a vital part of the service to provide Transport, Respite Care and the delivery of meals on wheels. Please contact your local office if you are available to provide any assistance. Fees - are charged for all services, to assist with ongoing costs.
